Here's a basic Checklist to be  prepared for your tax appointment

to ensure that you receive all the tax credits and deductions allowed.

As you begin receiving important tax documents in January, file them with this checklist.

  1. Name

  2. Social Security Card / ITIN / EIN

  3. Last Year’s Tax Return or Last Year Adjusted Income if new Client only

  4. Valid Driver’s License / ID

  5. Dependents Birth Certificate if new Client.  


  1. All W-2’s

  2. Bank Interest (form 1099-INT or substitute)

  3. Unemployment Compensation (form 1099-G)

  4. Health Connector Insurance Coverage (form 1095-A)

  5. Self-Employment Income and Expenses (Uber online report), (Total millage), (Bank Statements), (Expenses Report) (1099-NEC)

  6. Child Care Provider: Name, Address, EIN and Total Amount

  7. Gambling or Lottery Winnings     (W-2G for some winnings)

  8. State Income Tax Refund (1099-G)

  9. Pension Income (Form 1099-R),

  10. Social Security Benefits (SSA)

  11. Estimated Taxes Paid Already

  12. Social Security or Railroad Retirement (SSA-1099 or RRB-1099)

  13. IRA or 401(k) Distribution (1099-R)

  14. Dividend Slips (form1099-DIV or substitute)

  15. Stock Sales (form1099-B or Broker Statement)


  1. Personal Property taxes

  2. Charitable contributions

  3. Gambling Losses

  4. Military Moving Expenses

  5. Traditional IRA Expenses

  6. Higher Education Expenses / College (Form 1098-T)

  7. Student Loan Interest paid (form 1098-E)

  8. Pension Income (Form1099-R)

  9. Educator Expenses

  10. Medical Expenses


  1. Mortgages Interest (form 1098)
